Nurse Introduction
Hello and Welcome to the Academy at Palumbo!
My name is Jane Cruice (Nurse Jane) and I am the full-time Certified School Nurse at Palumbo. On Mondays and Fridays we have an additional nurse, Cheri Graham (Nurse Cheri). As we begin the school year, you will find direct links to all the necessary forms on this page that you may need to have completed at your student’s health care appointments.
For All Students
Each year, the district and state require that every student complete the Student Emergency/Medical information form (S865.) This form provides valuable updates on your student's health conditions as well as provides permission for your student to receive Tylenol and/or Ibuprofen during the school day. Without this form completed, we are unable to provide these medications to the student during the school day. You can find the S865 Form here.
12th Grade Students
For students entering the 12th Grade, there is a second Meningitis vaccine/booster required. Please be sure to make an appointment with your student’s health care provider and have them receive this vaccine if they have not already done so. Please be sure to email jcruice@philasd.org with proof of the vaccines received.
All students are required to be up to date with their immunizations. View district and state requirements, here.
Physical Examinations
Incoming students and all 11th Grade students are required to have an updated physical completed by their primary care provider. Please be sure to schedule an appointment with your student’s primary care provider and have the form—MEH1—completed at the visit.
For Students who require medication in school:
Any student that is required to take medication during the school day, such as: asthma inhalers, diabetes medications, ADHD or seizure medications, etc., they must have the MED1 form completed by their primary care provider and brought to school with the prescribed medication.
All medication MUST be in the original box from the pharmacy with the student's name.
