Please let me introduce myself, my name is Christine Stevens and I am the nurse at The Academy of Palumbo. Each school year, the Medical Emergency form and the Covid testing consent must be completed for your student. You can complete the forms electronically, see links below, or by paper. If you would like to complete the form by paper, feel free to obtain the forms from the main office or the health room. The forms and electronic links are also available on the School District of Philadelphia’s website: www.philasd.org.

Please return the completed forms on the first day of the school year 2023-2024.
